Pathways to Report Incidents or Concerns
Tell Someone allows all members of the Creighton community to use simple online forms to report harassment, discrimination, grievances or concerning behavior. Any information provided in these forms will be private; Creighton staff members who receive Tell Someone forms have signed a non-disclosure agreement—committing them to maintain the highest level of privacy.
These forms should NOT be used in an emergency. At the Omaha campus, you can report an emergency or suspicious activity, by calling Public Safety at 402.280.2911. At all other locations, please dial 911.
About the Forms
Tell Someone forms are tailored to different types of concerns or campus locations. You will find links to forms for specific concerns in the section below. You can access these forms with your NetID and Blue password. Currently, there is one form for the Phoenix campus. All other forms are for the Omaha campus.
Reporting Forms
Forms for Staff Members
The following forms exist for only specific types of staff members. If you are not a staff member described in the list below, please use the links above for reporting.
- Department of Residential Life Report Form: This form can be used by Residential Life staff, including Resident Assistants and Resident Directors, to report concerns.
- Student Leadership and Involvement Center Report Form: SLIC staff and student leaders, including fraternity and sorority presidents and executive board members, can use this form to report concerns.
- Campus Security Authority Report Form: Trained Campus Security Authorities (CSAs) can use this form to report concerns.
